The thesis consists of introduction, 5 chapters, conclusions, practical recommendations, bibliography with 202 references, 2 annexes, on 212 pages of main text, 11 figures and 50 tables. The obtained results were published in 44 scientific articles.
Domains of research: human and animal physiology, cardiology, paediatrics. Aim of research: Analysis and estimation of evolution of chronic myocardial dysfunctions, secondary to myocarditis and arterial hypertension, metabolic pathological processes and pathways, in children and adolescents, also the impact on the physiological integrative state of the heart and their therapeutic correction.
Objectives of research: Estimation of evolution of chronic myocardial dysfunctions, secondary to myocarditis and arterial hypertension, evaluation of biochemical parameters in patients with chronic myocardial dysfunction depending on the treatment strategy, appreciation of dynamics in hormonal and biochemical index changes in treatment outcomes, comparative analysis of treatment efficacy with angiotensin conversion enzyme inhibitors and inhibitors of aldosterone receptors, in patients with chronic myocardial dysfunctions, secondary to myocarditis and arterial hypertension, elaboration of the necessary concepts and algorithms in diagnosis and treatment options. Novelty and originality of research: there was elaborated an relational complex study regarding the physiological, metabolic and clinical evaluation of children and adolescents with chronic myocardial dysfunctions; estimation of evolution of chronic myocardial dysfunctions, secondary to myocarditis and arterial hypertension, identifying new metabolic markers in diagnosis of chronic heart failure, and therefore to elaborate new schemes and concepts for the efficiency of diagnosis and treatment of chronic myocardial dysfunctions.
Main new results for science and application: there were identified the characteristic modifications of the morphological and functional state of the heart in correlation with certain metabolic modifications in children and adolescents with chronic myocardial dysfunction, during the evolution of the pathology and the treatment directions, that allowed to underline the specific clinical and paraclinical data congruence, also pathological pathways, and therefore to elaborate new schemes and concepts for the efficiency of diagnosis and treatment. Tthere were identified the specific metabolic pathways that describe and proper evaluates the evolution of chronic myocardial dysfunctions, also new biomarkers and their acitivity measurments for early and correct diagnosis, and the preparation of new concepts and algorithms of diagnosis procedures and treatment orientation.
Theoretical significance: consists of the new information acquired
from the results of the research, namely the functional and morphological changes of the heart in
children and adolescents with chronic myocardial dysfunction and the specific metabolic index,
together with the elaboration of new relevant schemes for diagnostic and therapeutic criteria.
Applyed value of the research: identifying new markers for evaluation of chronic dysfunction
of the myocardium in children and adolescents, elaboration of new recommendations and
schemes for resultative and efficient guidelines in diagnosis and treatment measures, within the
studied nosologies.
Implementation of the scientific results: The results of the study were
approved as trustful scientific background and have been implemented in the Republican
Pediatric Clinical Hospital “Emilian Cotaga”, in Pediatric Clinical Hospital “Valentin
Ignatenco”, in the Institute of Scientific Research for Mother and Child Care, in the Hospital of
Glodeni, State University of Medicine and Pharmacy “Nicolae Testemitanu”.
