The thesis is exposed on 173 pages and consists of introduction, 4 chapters, conclusions, practical recommendations, bibliography from 195 sources. Illustrative material includes 42 tables and 24 figures. The obtained results are published in 14 scientific works.
Domain of research: cardiology.
Goal: Evaluation of the effectiveness of statins in the prevention and treatment of coronary alterations after interventional procedures with stent implantation.
Objectives of research: Determination of the altered myocardial markers after coronary angioplasty with stent implantation. Dinamics of markers of oxidative stress, endothelial dysfunction and inflammation in the immediate and late period after coronary angioplasty (PCI). Assessment of the effects of different doses and the application ways of statins and the influence of the type of stent used (metal or coated) on appointed markers. Evaluation of major cardiovascular events in the immediate and late period after coronary angioplasty.
Novelty and originality of research: The study brought further evidence on pathogenetic mechanisms targeting vascular alterations caused by coronary angioplasty, the role of oxidative stress and inflammation in the evolution of this process. In this study, for the first time in our country, attempts of myocardial and vascular periprocedural protection with various schemes and doses of statins administration have been made, including locally (stent coated with statins).
Scientific problem solved: Coronary angioplasty is accompanied by an increase of parameters of oxidative stress, activation of inflammation and endothelial dysfunction.
Theoretical value: Evidence of the positive role of statins on oxidative stress, inflammation and endothelial dysfunction parameters, and on major cardiovascular events have been obtained.
Applied value: High doses, 80 mg given 12 hours before the angioplasty, as well as local action of simvastatin are more effective than low doses in reducing oxidative stress and inflammation development.
Practical implementation: The study results were estimated as scientific valuable and have been implemented in current curative activity of the Institute of the Cardiology.
