Summary

Fala Valeriu “Complex treatment of patients with chronic periodontitis and edentulism” Thesis of Doctor of Medical Sciences Structure of the thesis: the work contains 200 pages of typed text and consists of an introduction, 6 chapters, conclusions, recommendations, bibliography that includes 375 references, summaries in Romanian, English and Russian, 2 annexes. There are used 58 figures and 48 tables. The achieved results are published in 53 scientific papers. Key-words: gingivitis, periodontitis, edentulism, diagnosis, complex treatment, optimization, preclinical disorders, myocardial index. The field of the study and the objectives of the thesis: optimization of complex treatment of chronic periodontitis, partial and complete edentulism, is based on the interdisciplinary diagnosis with complex therapeutic action on the level of stomatognathic system (1), overall health (2), metabolic disorders associated with the periodontal pathologic processes (3), periodontitis-target organs (4). Following the aim of the study, the optimization of the complex treatment of chronic gingivitis and periodontitis, partial and complete edentulism, in association with BioR®, on the basis of interdisciplinary study through the evaluation of correlations of clinical manifestations of chronic generalized periodontitis with the preclinical ischemic myocardial processes; the study of correlations between the alveolar bone density, quantitative parameters (thickness) of the Schneiderian membrane, the degree of indentation in the maxillary sinus, based on the severity of chronic periodontitis; the analysis of the metabolic systemic processes (in vivo) and in hepatic and cardiac tissues (in vitro), under the influence of BioR®; the experimental and clinical study of the processes of osteoregeneration/osseointegration using the graft materials in association with BioR®; the comparative analysis of the occlusal rehabilitation methods in patients with edentulism, in correlation with the subjective indices of occlusal rehabilitation. The novelty and scientific originality: for the first time there was revealed the quantitative peculiarities of the Schneiderian membrane in accordance with alveolar bone characteristics, the indentation degree and severity of periodontitis; there was established the efficiency of BioR® in patients with gingivitis and chronic generalized periodontitis on clinical, functional, morphological, metabolic levels; in an in vitro research, for the first time, there were systematized peculiarities of BioR® action on metabolic level in hepatic and heart tissues; there was optimized the rehabilitation of occlusion on the basis of personalized indices; there was supported a new concept periodontitis–target organ (heart). The newly obtained results, relevant to science and practice that have determined the creation of a new scientific direction or a solution of problems that bear a scientific and practical nature or of a major importance problems are based on the initiation of multidisciplinary cooperation, for the optimization of the complex treatment of chronic generalized periodontitis and edentulism, on the basis of therapeutic action on the stomatognathic system, overall health, metabolism and the activity of target organs. Theoretical value: there was enhanced the knowledge referring to the manifestation and evolution of chronic generalized periodontitis and edentulism (the relationship between the dental-periodontal structures, alveolar bone and maxillary sinus); there were described modulatory metabolic effects – stimulation and reduction in hepatic and cardiac tissues under the influence of BioR®; there was developed the concept of periodontitis–target organ (heart) – a new theoretical support in diagnosis and complex treatment of chronic periodontitis and edentulism associated with preclinical ischemic myocardial disorders. The practical value of the work consists of the demonstration of BioR® efficiency in accordance with the severity of the disease and the routes of administration (spray, intramuscular, intragingival); identification of preclinical signs on the stomatognathic system level and on the cardiac system level in patients with chronic generalized periodontitis and edentulism; optimization of the complex treatment for chronic periodontitis and edentulism with simultaneous action on the stomatognathic system, overall health, metabolism and preclinical ischemic myocardial disorders.
