Summary

The thesis consist of 131 pages, beginning with introduction, 5 chapters, general conclusion, practical recommendations and bibliography from 221 sources. The results of this original research are published in 17 scientific papers.

Field of research: cardiology.

Purpose of research: To evaluate the antihypertensive and cardioprotective effect of angiotensin II receptor blocker (ARB II) Eprosartan and angiotensin-converting enzyme (ACE) inhibitor Ramipril in essential arterial hypertension.

Objective of research: To study the diurnal blood pressure pattern in hypertensives. To establish the influence of long-lasting therapy (Eprosartan versus Ramipril) on blood pressure, diastolic function and left ventricular remodeling. To determine the correlation between diurnal blood pressure profiles, diastolic dysfunction and left ventricular remodeling.

Innovation and originality of research: The study certificates the beneficial influence of Eprosartan on blood pressure variability, cardiac remodeling and diastolic dysfunction. Scientific problem: to establish the antihypertensive and cardioprotective efectiveness of Eprosartan versus Ramipril in order to complete the existing pharmacological strategy with efficient scheme of blood pressure control, inducing left ventricular remodeling and diastolic dysfunction.

Academic value: The results of study emphasize the efficacy of both Ramipril and Eprosartan on blood pressure control, left ventricular remodeling and diastolic dysfunction in hypertensive patients. However, datas from this research report a better reduction in systolic blood pressure, more potent regression of left ventricular remodeling and left ventricular lusitropy's normalization in patients randomized to treatment with ARB II Eprosartan.

Practical value: The results of these research will be applied to improve treatment strategy in patients with essential hypertension and subclinical target-organ damage - cardiac remodeling and diastolic dysfunction of left ventricle.

Practical implementation: The results of the research gained academic appreciation and have been implemented in the clinic of the Institute of Cardiology, Сlinical hospital of municipality "Sfînta Treime"(Afiliation of Department of Cardiology), and State University of Medicine and Pharmacy “Nicolae Testemitanu” (Department of Cardiolgy).
