"Surgical treatment of myopia and myopic astigmatism using the LASIK method with formation of ultrathin flap". MD Ph. D thesis.
The work consists of 121 typed pages and include: introduction, four chapters (literature review, materials and methods, two chapters with results of proprietary research), conclusions and practical recommendations, annotations, bibliography of 206 sources, 16 annexes, 49 figures and 15 tables. The results are published in 12 scientific works and 9 without co-authors.
Keywords: myopia, myopic astigmatism, refraction, spherical equivalent, ultrathin corneal flap, LASIK.
Domain of research: ophthalmology.
Goal of research: Evaluation of clinical and functional outcomes in LASIK refractive surgery with formation of corneal flap less than 110 μm in patients with myopia and myopic astigmatism.
Objectives of the study. Evaluation of keratorefraktometry and corneal topography in patients with myopia and myopic astigmatism after LASIK surgery with a corneal flap less than 110 μm. Assessment of clinical and functional results and recovery period in patients with myopia and myopic astigmatism, after the refractive laser correction LASIK with the corneal flap less than 110 μm, compared with the LASIK procedure with the corneal flap more than 110 μm in patients with corneal pachymetry more than 500 μm. Evaluation of results of surgical refractive laser correction LASIK with the corneal flap less than 110 μm in patients with corneal pachymetry less than 500 μm, inaccessible for the refractive laser correction LASIK with a corneal flap more than 110 μm. Elaboration of an optimal algorithm for examination, treatment and monitoring of patients after refractive surgery with LASIK method in patients with myopia and myopic astigmatism, including the initial corneal thickness less than 500 μm. Determination of prognostic criteria of the results LASIK procedure with the corneal flap less than 110 μm in patients with myopia and myopic astigmatism.
Research methodology. This paper is a clinical, analytical and prospective research based on a wide range of methods and techniques. Study subjects were randomly selected after using the inclusion and exclusion criteria.
Solved scientific problem in the study is to determine the efficacy and safety of LASIK procedure in the correction of myopia and astigmatism with the formation of the corneal flap less than 110 μm in comparison with the LASIK procedure with the formation of the corneal flap over 110 μm in patients with corneal pachymetry more than 500 μm and determining the effectiveness of LASIK with the formation of the corneal flap less than 110 μm in patients with corneal pachymetry less than 500 μm in order to achieve predicted results.
Theoretical significance of the work. The resulting study data demonstrate the effectiveness of the correction of myopia and myopic astigmatism by LASIK with the formation of corneal flap less than 110 μm to achieve in a short time high refractive and functional results. The resulting research data on the dynamics of objective refraction, corneal topography, pachymetry, served as the basis for developing an algorithm of follow-up and management of patients. The data obtained in this research complement the available literature data on the surgical correction of myopia and myopic astigmatism in patients with thin corneas. Reliable statistical data were obtained demonstrating efficacy, safety, stability and predictability of the LASIK correction of myopia and myopic astigmatism in patients with thin corneas.
Applicative value. Was developed an algorithm for patient selection, indications and contraindications for the correction of myopia and myopic astigmatism by LASIK with the formation of corneal flap less than 110 μm. Proposed a scheme pre- and post-LASIK medical actions for patients with myopia and myopic astigmatism with the formation of corneal flap less than 110 μm. Were shown the effectiveness and predictability of the correction of myopia and myopic astigmatism in patients with thin corneas. Were evaluated and demonstrated the benefits and effectiveness of the LASIK method with the formation of corneal flap less than 110 μm and the results of its impact on the quality of life of patients.
Implementation of scientific results. The methods developed in the study were implemented in the Medical Center "Eye Microsurgery" in Chisinau, Republic of Moldova.
