The structure of the thesis work is presented on pages of printed text, consists of an introduction, 5 chapters, conclusions, practical recommendations, summaries in Romanian, Russian and English, andbibliography with 201references, including 40figures,43 tables, 3 formulas and 7 annexes. The purpose of the study: Surgical treatment optimisation based on oncological prognosis of percutaneous imaging guided biopsy, including the effectiveness assessment of vertebroplasty in patients with vertebral tumors.
Research objectives: Specification of indications, contraindications, methodology, significance and veracity, and influence of percutaneous biopsy on surgical management of spinal tumors; indication’s analysis, improvement of methodology with assessment of early and late outcome of vertebroplasty in patients with lytic spinal tumors; development of an algorithm for diagnosis and differentiated surgical treatment of patients with spinal tumors.
Scientific novelty and originality of the study consists in efficacy analysis of percutaneous biopsy in the diagnosis and management planning of patients with spinal tumors; has been improved and introduced into practice imaging method for necessary PMMA volume assessment for the vertebroplasty; vertebroplasty efficacy has been demonstrated in the surgical treatment of patients with malignant and benign lytic vertebral tumors with analysis of early and late outcome.
Solved scientific problem: has been established efficiency of percutaneous biopsy in surgical management of patients with spinal tumors, has been estimated the value of vertebroplasty in the pain treatment of patients with spinal lithic tumors. Has been developed imaging method for necessary PMMA volume assessment for vertebroplasty in the prophylaxis of intra- and post-procedural complications.
Theoretical significance of the study. Results enabled improvement of diagnostic methods, improving the quality of life of patients with limited life expectancy.
Applicative value of the study. Has been developed the research based algorithm for diagnosis and treatment of patients with spinal tumors.Scientific results implementation.
Implementation of scientific results in clinical practice of Institute of Neurology and Neurosurgery of Moldova, SCR and IO, resulted in treatment of large number of patients with spinal tumors.
