Summary

The thesis contains 125 pages of main text divided into: introduction, 4 chapters, conclusions and recommendations, bibliography of 203 sources, 44 figures, 23 tables and 9 appendices. The results of the study were published in 45 cientific papers.

Domain of research: Rheumatology.

Aim of the study: to evaluate cutaneous expression and it’s role in evolution of systemic lupus erythematosus in order to elaborate a model of patient’s management.

Study objectives: to describe the cutaneous involvement in patients with SLE; to quantify the activity and severity of skin involvement using CLASI Index; to assess the efficacy of the complex treatment, in association with hydroxychloroquine on skin involvement in SLE; to study the influence of cutaneous manifestation on patient’s quality of life; to elaborate a model of management of patients with SLE and cutaneous involvement.

Scientific novelty and originality: the study is remarkable by a plural approach on cutaneous involvement in SLE patients and was conducted according to a dual design: transsectional and prospective study. In the research there were studies clinical and paraclinical particularities of SLE patients with skin involvement, using validated clinical tools. O novel approach was the determination of the efficacity of hydroxychloroquine on cutaneous manifestations in a prospective, randomized trial.

The scientific problem solved in the thesis: elaboration of a model of personalized approach to patients with cutaneous involvement and SLE, which has contributed to the optimization of their clinical management, in order to apply it in further clinical practice.

Theoretical importance of the research: the study established the types of skin involvement using sensible clinical tools, also, the research issued an algorithm of treatment, and by this highlighted the holistic approach in patients with SLE and skin involvement.

Applicative value of the work: the results of the study have argued the validity and informativity of clinical tools in the complex assessment of patients with SLE and skin involvement. Also, the research contributes to the study of the efficacy of hydroxychloroquine on cutaneous manifestations.

Practical implementation: The results were included in the statement of National Clinic Protocol on SLE and in the training process at the Department of Internal Medicine.
