Summary

Thesis structure: introduction, five chapters, general conclusions and recommendations, 298 bibliographical sources, 6 annexes, 231 pages of main text, 90 figures, 53 tables. The results are published in 37 scientific works, one patent obtained.

Aim: Optimization of implant-prosthetic rehabilitation of patients with severely atrophic posterior edentulous regions of maxilla by elaborating minimal invasive procedures of endosseous dental implant placement.

Objectives: The evaluation of success and survival rates of implants placed by standard method in posterior region of maxilla; Postoperative evaluation of patients after sinus grafting procedure through lateral access; The evaluation of success and survival rates of implants placed with simultaneours sinus grafting procedures with lateral access in posterior atrophied areas of maxilla; The evaluation of success and survival rates of implants placed delayed in sinus grafting procedures with lateral access in posterior atrophied areas of maxilla after graft healing; Elaboration of a minimally-invasive method of two-stage implant placement with maxillary sinus floor elevation through crestal approach; Elaboration of a minimally-invasive method of one-stage implant placement with maxillary sinus floor elevation through crestal approach; Postoperative evaluation of patients after sinus grafting procedure with elaborated method; Elaboration of radiological method of periimplant bone evaluation in cases of implant placement with sinus lift procedure. The monitoring of intrasinusal bone modeling/remodeling depending on the method of sinus floor elevation; The monitoring of crestal bone modeling/remodeling depending on the method of sinus floor elevation; Evaluation of short implants in crestal sinus lift procedure. Highlight, prevention and treatment of complications occurred during implant placement with sinus lifting procedures.

Scientific novelty: It has been demonstrated that flapless dental implant placement through crestal approach in posterior maxillary atrophied areas without bone condensation, a new intrasinusal periimplant bone is formed bases on blood clot and bone fragments and it is modeling/remodeling under the influence of masticatory forces and air pressure from maxillary sinus. New results for science and practice: creation of a new concept for protection of physiological reparatory forces in order to facilitate the osseointegration process of implants, this led to elaboration of two new methods of implant placement with further optimization of implant-prosthetic rehabilitation of patients with posterior edentulous maxilla.

Theoretical importance and applicative value: The importance, necessity and indications for minimal invasive implant placement in prosthetic rehabilitation of patients with severe atrophied posterior edentulous maxilla have been argued and demonstrated.

Implementation of scientific results: The results of the study have been implemented into the educational process of students, residents and doctors at the department of oral and maxillo-facial surgery and oral implantology „Arsenie Guţan” of the Public Institution State University of Medicine and Pharmacy „Nicolae Testemitanu” as well as in private clinics: “F. Atamni”, „M. Atamna”– Israel, S.R.L. Masterdent, Omnident, Gumeniuc-Dental – Chișinău.
