Field of study: Medicine The purpose of the research is to develop a new method in the treatment of chronic pelvic pains by the pericervical use of the Lidocaine, Dexamethasone and B1, B6 vitamins drug complex and the dynamic elucidation of the free radicals level during 5 years, based on the study of antioxidant activity by Raman spectroscopy.
Objectives of the study: Determination of the anamnestic-clinical particularities, characteristic for the occurrence of chronic genital pelvic pains in the study group; elaboration of a new method of treatment, with the use of pericervical therapeutic drug complex; estimating the clinic-evolutionary efficacy of the proposed treatment for gynecological patients resistant to standard treatment; evaluating dynamically over 5 years of the level of antioxidants in the use of treatment with the therapeutic drug complex as compared to standard treatment in patients with chronic pelvic genital pain by Raman spectroscopy.
The scientific novelty of the obtained results: A complex, comparative, prospective, longitudinal clinical, paraclinical research was carried out on a representative group of patients, which allowed for the first time: to elucidate some anamnestic and clinical-evolutionary features, characteristic of the occurrence of chronic pelvic genital pains; to elaborate a new method of treatment, with the pernicious application of the drug complex; to perform a comparative research of gynecology patients, using the methods of standard therapy and pericervical therapy with the Lidocaine, Dexamethasone and B1, B6 vitamins drug complex, proposed by the author; to establish that the Raman spectroscopy method can serve as a criterion for objectivizing the degree of activity of the antioxidant system in the body as well as for determining the curative strategy to the detriment of more laborious, invasive and costly investigations; to demonstrate the possibility of pursuing the evolution of a chronic pelvic genital pain and the effectiveness of differential treatment in dynamics by elucidating the antioxidant activity of the organism by Raman spectroscopy.
The important scientific problem solved in the thesis consists in the elucidation of some anamnestic and clinical-evolutionary particularities characteristic of the occurrence of chronic pelvic genital pain, the elaboration of a new treatment method by the pericervical use of the therapeutic drug complex, which allowed the rapid socio-professional insertion of the patient, normalization of clinical picture, decrease of free radicals, confirmed by their appreciation with Raman spectroscopy.
Theoretical significance. The present study updates the data related to the diagnosis and treatment of chronic pelvic genital pain, theoretically and practically argues the impact of our developed and proposed method, based on the use of the Lidocaine, Dexamethasone and B1, B6 vitamins drug complex, which deserves to be implemented in the respective field. The results of the study complete the knowledge of physicians and specialists with regard to the treatment of gynecology patients with chronic pelvic genital pain and allow the assessment of the state of the antioxidant system in the body by noninvasive Raman spectroscopy.
Applicative value of the paper. The practical importance of the present scientific research is the development of a new method of treatment of chronic pelvic genital pains algal with the pericervical use of the therapeutic drug complex. Additional criteria were recommended to objectify patients' status, to predict disease progression and to evaluate the effectiveness of therapy based on Raman spectroscopy assessment of free radicals. For the first time, a complex therapy of chronic pelvic genital pains, used pericervicaly, was developed, which allows both the prophylaxis of chronic inflammation and the reduction of inflammatory-destructive processes.
Implementation of scientific results. The main results of the study were implemented in the current curative activity of the Gynecology Service of the Sighetu Marmatiei Municipal Hospital with addressability from Maramures and the gynecology departments of Iasi, Cluj-Napoca, Timisoara from Romania, the Mother and Child Institute and the Extramed Clinic in Moldova.
