Thesis structure: introduction, four chapters, general conclusions and recommendations, 198 bibliographic sources, 3 annexes, 118 pages, 33 figures, 13 tables. The results were published in 18 scientific papers.
Field of Study: Dentistry – 323.01 Aim: Evaluation of the clinical and paraclinical features of peri-implant disease to highlight evolutionary features during non-surgical treatment and to optimize medical management in their prevention.
Objectives: Determining the quality of the accuracy of the conical beam computed tomography for the detection of peri-implant bone lesions compared to the contact radiograph around the implants diagnosed with peri-implant diseases; analysis of the efficacy of the use of sodium hypochlorite compounds in the non-surgical treatment of peri-implant diseases; evaluation of inflammatory markers and periodontopathogenic bacteria in relation to the diagnosis and results of the non-surgical treatment of peri-implant diseases; elaboration of an algorithm for prevention and treatment of peri-implant diseases.
Scientific novelty and originality: Comparative analysis of peri-implant bone resorption on conical fascicle computed tomography and contact radiography in patients diagnosed with periimplant diseases. Use of immunological and microbiological tests of the fluid of the peri-implant space in the peri-implant diseases, at the diagnosis stage and at different stages after treatment. The complex research (clinically, radiologically, microbiologically and immunologically) of the action of chlorine compounds in the treatment of peri-implant disease, around the screw-retained implant-prosthetic constructions.
Scientific problem solved: Elaboration of a complex algorithm for prevention and treatment of peri-implant diseases, by using chlorine compounds in non-surgical treatment, and modern diagnostic methods: radiological, clinical, microbiological and immunological.
Theoretical importance: Enhancing knowledge regarding the etiology and diagnosis of periimplant diseases in order to make the primary prevention measures more efficient and to establish the correct diagnosis. Clinical and microbiological efficacy of the use of chlorine compounds in the treatment of peri-implant diseases has been demonstrated. As a result of the determination of the integral cytokinik index, threshold values were determined in the predictability of the non-surgical treatment. Based on the radiological calculations it was determined that in the vestibulo-oral sense the bone resorption on the cone-beam computed tomography is higher, than the mesio-distal on the contact radiography. The results obtained in the study constitute a theoretical basis in the complex management of peri-implant diseases, which contributed to the development of a new algorithm for diagnosis, prevention and treatment.
Applicative value of the work: Optimization of the diagnostic process by using modern methods in patients with peri-implant diseases will allow the dentist to personalize their management and and shorten the period from non-surgical to surgical resective or regenerative one. The use of chlorine compounds in the treatment of peri-implant diseases, allowed to reduce the bacterial quantity in the peri-implant spaces of Gram-negative microflora and to reduce the inflammatory clinical signs Implementation of the scientific results: Scientific results have been implemented in the process of research and clinical activity in D.C. ,, Fala Dental’’, D.C.. ,, Megalux Dent’’ and The chair of therapeutic stomatology.
