SUMMARY
SARBU Oxana, „Features of the evolution, quality of life and prognosis of ankylosing spondylitis in women”, doctoral thesis, Chisinau, 2022.
The work is exposed on 110 pages of electronic text and includes introduction, literature review, 3 chapters, conclusions and practical recommendations, bibliography from 200 literary sources. The thesis is illustrated with 35 tables, 21 figures, 14 annexes. The results were presented in 22 scientific publications.
Key words: ankylosing spondylitis (AS), clinical and paraclinical predictors, HLA-B27, early diagnosis algorithm.
The aim of the study was the comparative research of clinical manifestations at the beginning and in the manifest stage, immunogenetic and imaging alterations (radiological and by MRI), quality of life and prognosis, of female and male patients in AS in order to propose criteria for early diagnosis of ankylosing spondylitis in women.
Objectives of the study. 1. Gender comparative analysis of clinical manifestations at the onset and manifest state of the disease in females and males in AS. 2. Gender comparative research of radiological indices and MRI of the disease in the onset and manifest stage and their correlation with the HLA-B27 histocompatibility antigen in AS. 3. Comparative assessment of quality of life in women and men with ankylosing spondylitis. 4. Assessment of the prognosis of ankylosing spondylitis in women compared to men. 5. Proposal of the algorithm for early diagnosis of ankylosing spondylitis in females.
Scientific novelty and originality. The analytical study assessed the features of the evolution, the articular and extra-articular manifestations, the quality of life and the prognosis of ankylosing spondylitis in women..
The important scientific problem solved in the thesis. Assessment of the clinical and evolutionary features of ankylosing spondylitis in women in conjunction with the estimation of paraclinical predictors to strengthen an algorithm for diagnosis and prognosis of the disease, as well as approach to personalized treatment.
The theoretical significance of the thesis. By highlighting the clinical, imaging, immunological and genetic landmarks, an important evidence is provided in the conceptual prerogative of the evolution of ankylosing spondylitis in women in onset and manifest stage.
The applicative value of the work. The determination of the early signs and symptoms characteristic of these diseases, in relation to the immunological, immunogenetic and imaging changes of the onset is necessary, in order to guide towards an early diagnosis of ankylosing spondylitis in women and personalized treatment. In this sense, the work is, for the first time, a comprehensive study of the characteristics of ankylosing spondylitis.
Implementation of results. The results of the study were implemented in the teaching process of the Discipline of Rheumatology and Nephrology of Nicolae Testemitanu SUMPh, as well as in the practical activity of the Rheumatology department of PHI Holy Trinity Municipal Clinical Hospital and the arthrology and rheumatology departments of PHI Republican Clinical Hospital
