Summary

Field and goals of study. A total of 248 patients with benign lesions of vocal fold were included in this study. 135 patients were treated by traditional endolaryngeal microsurgery and 113 of them – by using CO2 laser. An animal study with postoperative histological evaluation of vocal cord`s mucosa in 10 dogs was performed.

The novelty and scientific originality. The histological evaluation of dog`s mucosa in 2 weeks and 2 months after surgical intervention demonstrated more severe cicatricial modification in the cases treated with CO2 laser in comparison with those treated by traditional method. Traditional method of surgical intervention contributed for recovery of integrity of stratifical scuamos epithelium. In cases treated with CO2 laser we identified nodules in collagen fibers with cicatrisation of lamina propria in its superficial and profound parts.

Theoretical signification and practical application. The results of animal study and laryngeal endoscopic microsurgery used on the patients with benign lesions of vocal fold determined the better efficiency of traditional method, the faster recovery in some kind of pathology. Surgery with CO2 laser is more useful in such cases when the risk of hemorrhagic complications is higher.

The clinic-experimental results, the methods and surgical techniques analysis of vocal fold benign affections patients treatment permitted us concentrate on indications, techniques and limits of endoscopic microsurgical methods with ”cool” instruments or CO2 laser.
