A Ph.D. thesis in the field of medicine „Differentiated treatment of urethral strictures” was performed by Scutelnic Ghenadie. An important prblem of modern urology was examined in this study – diagnostic and surgical management of urethral strictures (US). Scientifical research was carried out at the Urology and Surgical nephrology department of the State University of Medicine and Pharmacy „Nicolae Testemiţanu” in 2010. This scientifical work comprises 130 pages, including introduction, five chapters and reference list (223 sources), 17 tables and 39 figures.
The comparative study of different surgical methods implementaion in planned treatment of US in adult male patients was the aim of this study. This was reached by appreciation of the impact of different diagnostic modalities on the surgery method selection, as well as the study of histological modification influence on the results of surgical treatment, comparative analysis of classical and endourological approach to the correction of US, the studying of early and late complications. Accumulated data created the scientifical basis for diagnosis and planned treatment sequential algorithms elaboration.
This work is a prospective clinical analysis. Compared groups of patients were homogenous. „Case-control” method was applied for patients selection and the following data accumulation.
The scientific novelty of the thesis is underlined by determination of different diagnostic modalities (clinical, laboratory, instrumental, histological examination) role in formulation of the following surgical strategy, characteristics of both early and late complications development depending from the selected treatment method. Study results were reflected in treatment and diagnostic algorithms relevant to Moldavian clinical practice.
Scientifical and practical value of the thesis results from the validation of empirical indications for surgical treatment of US and determination of the operational sequence during all stages of the complex planned management of US, from primary diagnosis up to surgical treatment and postoperative management in case of US relapse.
Thesis material was published in 11 scientifical articles, the data being reported at 9 international and national conferences as well as implemented in praxis of the Urology and Surgical nephrology chair of the State University of Medicine and Pharmacy „Nicolae Testemitanu” (Urology departments of the Clinical Republican Hospital and National Scientifical Practical Center of Emergency Medicine).
