Summary

The paper contains 139 typewritten pages and includes: introduction, the list of literature, material and methods, 2 chapters containing the results of the author’s own researches, general conclusions, practical recommendations, bibliography citing 208 literary sources, and appendix. The illustrative material: 13 tables and 65 figures. Twenty scientific articles were published based on this thesis.

Field of study: ophthalmology.

Purpose of the study: to evaluate the efficiency and to optimize the treatment and prophylaxis of progression of acquired uncomplicated myopia through physiotherapeutic treatment and refractive therapy applied separately and combined.

Objectives of the study: To perform a complex clinical-functional examination in dynamics of the visual analyser in patients with uncomplicated acquired myopia. To estimate the therapeutic efect of physiotherapy in patients with progressing uncomplicated acquired myopia. To assess the efficiency of refractive therapy applied during the treatment and prophylaxis of progression of uncomplicated acquired myopia. To estimate the efficiency of refractive therapy combined with physiotherapeutic modern methods – electropuncture and laser stimulation, for the treatment of uncomplicated acquired myopia and the prophylaxis of its progression. To study the therapeutic efect depending on the degree of myopia, the patient age, the family factor and the method of treatment applied. To analyse the obtained results and to identify the optimal method of treatment and prophylaxis of uncomplicated acquired myopia progression and to develop practical recommendations.

Scientific novelty of the obtained results. For the first time, physiotherapeutic treatment (electropuncture and laser stimulation) was applied in the ophthalmology practice. The combined treatment of uncomplicated acquired myopia by associating the refractive therapy with contemporary physiotherapeutic methods (electropuncture and laser stimulation of the ciliary muscle) was applied in the ophthalmology practice for the first time. It was established a correlation between the annual myopia progression gradient according to the spherical equivalent value and the annual myopia progression gradient according to axial length. It was proved the efficacy of the studied methods and developed practical recommendations for ophthalmologists.

Important scientific problem that was solved. The scientific problem solved by this thesis identified the most efficient therapeutic method of treatment and prophylaxis of the progression of uncomplicated acquired myopia, which allowed the development of an algorithm to select a treatment method depending on the level of myopia, the age and familial factor.

Theoretical significance. New objective data about clinical refraction dynamics and biometrical parameters of the eye were obtained as a result of refractive therapy and physiotherapeutic treatment applied both separately and combined. The data obtained as a result of the study complemented the works in the area of ophthalmology with special reference to the treatment of uncomplicated acquired myopia. It was established the importance and role of annual myopia progression gradient according to axial length in the prognosis of progression of uncomplicated acquired myopia. Real statistical data, which proved the efficacy of refractive therapy combined with physiotherapeutic treatment was obtained.

Applicative value. It was established the therapeutic efect of the physiotherapeutic treatment, the refractive therapy and the combined treatment in comparison with the application of optical correction in patients with progressing uncomplicated acquired myopia of mild and moderate degree depending on the age and familial factor. It was developed and explained from the pathogenic standpoint: the algorithms of selection of the treatment and of progressing uncomplicated acquired myopia depending on the degree of myopia, the age and familial factor. It was established that the most efective method of treatment and prophylaxis of progressing uncomplicated acquired myopia is the combined therapy. There were estimated and proved the advantages and efficiency of the combined method by slowing the progression of uncomplicated acquired myopia and improvement of life quality.

The main scientific results proposed for defence. The importance of myopia degree, the age and familial factor in the efficiency of treatment and prophylaxis of progressing uncomplicated acquired myopia. The particularities of spherical equivalent evolution and axial length, depending on the treatment applied, the degree of myopia, the age and familial factor. The comparative analysis of the outcomes obtained as a result of conservative methods of treatment and prophylaxis applied to stop the progression of uncomplicated acquired myopia. The implementation of direct correlation formula between the annual myopia progression gradient according to to the spherical equivalent and the annual myopia progression gradient according to axial length for the prognosis of untreated uncomplicated acquired myopia progression.

Implementation of the scientific results. The methods developed during the study were implemented in the activity of the Medical Centre “Oculus Prim” from Chişinău, Republic of Moldova. 7
