The purpose of the study: Studying the efficacy of Interferon-free antiviral treatment in patients with chronic viral hepatitis C. The study objectives: Evaluation of the efficacy of the direct-acting antiviral treatment: Sofosbuvir + Ledipasvir and Sofosbuvir + Daclatasvir in patients with chronic viral C hepatitis. Evaluation of the evolution of clinical data, biochemical, virological and other laboratory indices in patients with chronic viral C hepatitis at the beginning, at the end of the treatment and at 24 weeks after the antiviral treatment. Analysis of side effects and complications after treatment with direct-acting antiviral drugs in patients with chronic HCV. Scientific novelty and originality: For the first time in the Republic of Moldova, Interferon-free antiviral treatment has been used in patients with chronic viral C hepatitis. The efficacy of the treatment with direct-acting antiviral drugs: Sofosbuvir+Ledipasvir and Sofosbuvir+Daclatasvir compared to treatment with PEG-INF and Ribavirin was evaluated. Adverse reactions and complications were evaluated during and after completion of antiviral treatment. Results obtained: The obtained results, which contributed to solving the scientific problem, were implemented in the elaboration of antiviral treatment regimens for chronic viral hepatitis C. Interferon-free treatment resulted in the elimination of HCV and a sustained virological response much more frequently than treatment with PEG-INF and Ribavirin, which stopped the progression of hepatitis to cirrhosis and hepatocellular carcinoma, and also decreased liver fibrosis. The theoretical value: The obtained results solved characteristics of chronic HCV depending on age, sex, degree of liver damage and extrahepatic manifestations and allowed evaluating the effectiveness of different treatment regimens with direct action antiviral drugs. Practical value: The obtained results allowed the elaboration of the optimal schemes of antiviral treatment of chronic HCV, the elaboration of the criteria for selecting patients for antiviral treatment and for monitoring patients with chronic viral hepatitis C during the antiviral treatment and after its completion. The implementation of scientific results: The scientific results were applied in implementation of the National Program to combat viral hepatitis B, C and D for the years 2017-2021, were implemented in practice of the infectious doctors from the whole republic, in the studying process of the Departments of Infectious, Tropical Diseases and medical parasitology and of Infectious Diseases. 7 АННОТАЦИЯ Руссу Ирина. Область исследования: инфекционные болезни. Цель исследования: изучение эффективности противовирусной терапии без интерферонов у пациентов с хроническим вирусным гепатитом С. Задачи исследования: Оценить эффективность терапии препаратами прямого противовирусного действия: Софосбувир + Ледипасвир и Софосбувир + Даклатасвир пациентов с хроническим ВГС. Оценка клинических данных, биохимических, вирусологических и других лабораторных показателей у пациентов с хроническим ВГС в начале лечения, по окончании лечения и через 24 недели после окончания противовирусной терапии. Оценка побочных реакций и осложнений после лечения препаратами прямого противовирусного действия у пациентов с хроническим вирусным гепатитом С. Научная новизна: Впервые в Республике Молдова, пациенты с хроническим вирусным гепатитом С получили противовирусную терапию без интерферонов. Была изучена эффективность лечения препаратами прямого противовирусного действия: Софосбувир + Ледипасвир и Софосбувир + Даклатасвир по сравнению с лечением PEG-INF и Рибавирином. Во время и после завершения противовирусной терапии были оценены побочные реакции препаратов и осложнения. Полученные результаты: Результаты, способствовавшие решению важной научной проблемы, были включены в разработке схем противовирусного лечения хронического ВГС. Лечение без интерферонов чаще приводило к элиминации вируса гепатита С и регистрации устойчивого вирусного ответа чем лечение PEG-INF и Рибавирином, что привело к прекращению прогрессирования гепатита в цирроз и гепатоцеллюлярную карциному, а также и к уменьшению фиброза. Теоретическая значимость: Полученные результаты позволили отобразить особенности течения хронического ВГС в зависимости от возраста, пола, степени поражения печени и внепеченочных проявлений и оценить эффективность различных схем лечения препаратами прямого противовирусного действия. Прикладная значимость: Полученные результаты дали возможность разработать оптимальные схемы лечения хронического ВГС, критерии отбора пациентов для противовирусного лечения и мониторинга пациентов с хроническим ВГС во время противовирусного лечения и после его завершения. Внедрение научных результатов: Результаты исследований были использованы в Национальной программе по борьбе с вирусными гепатитами B, C и D на 2017-2021 годы и внедрены в практику врачей-инфекционистов по всей республике, в учебном процессе Кафедры инфекционных, тропических болезней и медицинской паразитологии и Кафедры инфекционных болезней.
