Summary

The present study, being dedicated to one of the most difficult branches of clinical oncology – Primary Multiple Malignant Neoplasm’s (TMPN) of hormone dependent organs (OHD), - generalize the results of thoroughly and complex prospective investigations of etiopathogenic aspects of this pathologies by patients from our country. It was established already, that risk factors can be various not only in different populations, but can change in the same population in results of social modification, ecologic statement etc., and possibly because of this cause the concepts directed on etiopathogenic aspects of TМNP of OHD provoke till present time controversial discussions. That is why we included in the study the analyses of various factors - biologic, constitutional, hereditary, reproductive function peculiarities, associated pathologies, hormone homeostasis peculiarities, diverse indices of immunity by patients with TMNР of OHD at diverse localization. The investigation was done based on prospective data of 305 oncology patients treated into IMSP Oncology Institute from Republic of Moldova in period of 2000-2005 years, which were distributed in two batches: the group I - 107 patients with CPM of OHD : a mammary gland, uterine corpus, ovaries, thyroid gland, the right segment of colon and II group – 198 patients with unilateral Brest cancer. That is why, in result of the investigations we can answer definitely to the final objectives of this study. For this we compared the risk factors with most weigh in prognoses of each polineoplasm’s associations with unilateral process among them and with each group of polineoplasm’s. Therefore, in primary multiple synchrony and metachrony cancer of mammary glandhave in common the following aspects: the use of drugs that affec the adenohypophysis, SNC affections, the increasing of ALT level, pathology of thyroid gland, the extending of first malign process and hepatic function disorders when addressing with first cancer. However, specific only for primary multiply synchrony cancer are: the increasing of level CA-15.3, and alpha –amylase, high frequency of lobular cancer. For primary multiple metachrony cancer of mammary gland the specific factors are: immunodepression, manifested by decreasing of active T lymphocytes, early age of addressing with I cancer and high frequency of genetic factor, compared with CU of MG. For Brest cancer associated with malignant process of localization are characteristic the same common factors for synchrony and metachrony Brest cancer: the increasing of ALT level, the using of drugs that affect the adenohypophysis, the high of level CA-15.3, high frequency of lobular cancer and in the same time appearance of new factors, which contribute to central hormonal disorders – the increasing of FSH level and increasing of frequency of renal pathology.
