Summary

We studied 100 (50 men and 50 women) cases of paranoid forms of schizophrenia, at whom the disease manifested till 25 years old (main group) and 100 patients (ollowing group) with schizophrenia, who have this disease till 26-40 years. Within this work it was used by us clinic-catamnesis and statistical examination technologies, formulating the data of questionnaire, including 135 most typical , emphasized features .

Those, who are taken ill with it till 25 years, are marked with pronounced polymorphism and atypism clinic- evaluative manifestation of disease.

At the beginning of the disease (in the age of 15-25 years) has a considerable progression. It can’t sustain classical variants during the paranoiac schizophrenia after Magnan (paranoiac, paranoid, paraphrenic).

The stages of the disease develop swiftly, it can’t bear their consequences, it is combined their symptomatology. As a rule, there is a lack of paranoiac stage and it is substituted by the accentual features that are specific for the patients with the syndromes: derealization – depersonalization, intoxication metaphysical syndrome, unfounded asthenic youthful syndrome, psychasthenia, neurosis – like, psychopath- like symptomatology.

Within the work it was studied provoked endogenous and genic factors of risk. The analysis of the material allows to determine the primary role of the endogenous factors in the beginning of the disease: inherited charge, patterns of the features.

At a certain stage they put a mechanism of the initial stage of disease, subordinated to the genetic predestination.

Among the enumerated genetic factors of risk of paranoid forms of schizophrenia statistically are found in the both groups (youthful and following): craniocerebral trauma, delivery at the women, lactation period, incomplete family, service in the army, abroad work, drug use, abuse of computer and Internet, standard and profile of education, somatic disease, previous beginning of paranoid schizophrenia.

Endogenous and genic factors are interconnected and interact each other, create necessary and enough conditions for the development of the beginning paranoiac form and influence especially on debut, clinic, evolution, and mainly on the polymorphism, atypism and pathomorphism during the disease among young people.

The first classified variants of the beginning of the disease among the group of young patients till 25 years in the early ages are: the beginning with “metaphysics intoxication”, the beginning with pseudo- neurotic states, the beginning with pseudo-psychopathic states, paranoid debut, acute beginning with hallucinate-paranoid and with paraphrene syndromes, beginning with depressive symptoms.

It is formulated the differentiation of variants of the beginning on the genital evidence and depending on the accentual features. It was examined the dependence between the factors of the risk, the variants of the beginning of the disease, structural identity and clinical disease among young patients.

The most typical variants are the following: pursuit, impact, poisoning, hypochondriac, religious attitudes. The delirium symptomatology is extremely polymorphous.

Each delirium appearance is associated with other states, forming many variants around the main delirium. The delirium is not inclined to the systematization, it has episodic character. Pursuit deliria are often encountered both men and women. Impact and religious delirium is often met among men, while hypochondriac and poisoning deliria among women.

Describing evaluative trait during the disease, most unfavorable prognosis of disease with early defect state begins in the first three years of the disease. Using the mathematical and statistical methods of data processing, we forecast most unfavorable during paranoid schizophrenia to the both groups of the patients (youthful and following).

It was revealed the signs, which bring to the early defect. These data can be used to determine the prognosis, most likely the appearance schizophrenia defect in the first three years of the disease.

The initial stages of disease are often hidden under a mask of other illnesses and carry nonspecific traits, being hardly discovered.

Using the research methods within work are the great experience for the diagnostics of the initial states of paranoid schizophrenia among the young patients till 25 years.

The obtained experience within work was systemized and is offering as diagnostical criteria of the initial manifestation symptoms and syndromes of paranoid schizophrenia
