Summary

Thesis structure: The dissertation paper consists of 160 pages that include: introduction, 4 chapters, conclusions, practical recommendations, bibliography consisting of 210 scientific sources, 145 pages of basic text. The work is illustrated with 21 tables, 88 figures and attachments. The results are published in 29 scientific papers, inclusive obtained two patents for inventions.

Field of research: 321.18 - orthopedics and traumatology.

The aim: study the frequency of spine deformities in school-age children and adolescents by the development and testing of a new organizational and functional screening model that provides early diagnosis and the use of methods of rehabilitation medicine.

The research objectives: Carrying out school spinal screening of children and adolescents and the identification of a group of persons with spinal deformities: defects of posture, scoliosis; identification of the main factors that caused the deformation of the spine, and their influence on the formation of the deformity severity; development of the algorithm of early diagnosis to identify risk for scoliosis and organization of monitoring; definiton of preventive and treatment and rehabilitation measures according to the type of spinal deformity.

Scientific novelty and originality. An efficient organizational and functional model of the system of specialized medical care for school-age children and adolescents with spinal deformities in the concept of preventive pediatric vertebrology was proposed and algorithm of early diagnosis to identify risk for scoliosis was elaborated. The incidence of children with spinal deformities: incorrect posture and scoliosis was determined in large collectives. The organization of mobile orthopedic cabinet was defined and approved with the functions of methodological center to identify school-age children and adolescents with spinal deformities and other musculoskeletal pathology in schools and the development of individual rehabilitation programs. The treatment method of progressive thoracic scoliosis was elaborated.

The scientific problem solved in the thesis consists of identification of main factors that contribute to formation and progression of spine deformities, argumentation of prophylactics concept by the development and testing of an optimal screening model that provides early diagnosis and the use of rational methods of recuperation medicine.

The theoretical significance and practical value. The proposed complex examination scheme of school-age children and adolescents including orthopedic clinical and instrumental examination, provides to determine risk for the possible development of spinal deformity, monitoring ensures the accuracy of diagnosis, prediction of the disease and helps to improve clinical and functional outcomes of pupils rehabilitation. The proposed monitoring system allows to monitor the effectiveness of spinal deformities correction in school-age children and adolescents and promptly adjusts tactics of treatment. This study tested and implemented a screening service, monitoring and specialized care for children with orthopedic deformities of the spine, which can be extended to all schools of the republic.

Implementation of scientific results. The school screening results are for information and for taking the necessary organizational measures for teachers, school health professionals, parents and pediatric orthopedists. Results are used in lecture courses and practical training at the Department of Orthopedics and Traumatology of State Medical and Pharmaceutical University "Nicolae Testemitanu".
